Who’s it for?
If you're a Westfund member who is covered by Gold tier or Silver tier Hospital cover with Extras cover (excluding Starter Extras) and have been living with one or more chronic conditions, you may be able to take advantage of this program.
Eligibility criteria
- Care Coordination Pilot Program is available to active, up to date Westfund members between the ages of 18 and 75 who have held Gold tier or Silver tier Hospital cover with Extras cover (excluding Starter Extras) for at least 12 months.
- Members must be living with one or more chronic condition, including but not limited to hypertension, arthritis, obesity, mental health conditions, cancer, heart disease or diabetes. The chronic condition must have been present for a minimum of six months and affects daily living.
- Please note additional clinical criteria may apply. Full program eligibility is determined by the program provider.
