Cardihab

Managing lifestyle risk factors is a crucial step in preventing cardiac events. However, we know accessing clinical support can be challenging, especially for those living in regional or rural areas. 

The good news? Personalised and flexible support is now available, enabling you to manage your heart health conveniently from the comfort of your home. You'll have all the support you need for better heart health, including your own blood pressure monitor.

What is it?

Cardihab offer programs that will help you manage your heart health, whether you're at risk of heart disease or are living with a heart condition.

The evidence-based digital solutions provide personalised, flexible options, making it more convenient and accessible to those who need support.

Cardihab offers two programs:

 

Who’s it for?

Primary Prevention is available to Westfund members who have any level of hospital or overseas cover and are at high risk of developing cardiovascular disease (such as having diabetes, high blood pressure, high cholesterol, obesity, or a strong family history), or have already been diagnosed with cardiovascular disease, and have not previously participated in the Primary Prevention Program. 

This program focuses on improving heart health by reducing the risk of developing heart disease or preventing heart disease from worsening.

It promotes lifestyle changes through education, monitoring (including a blood pressure monitor), and ongoing personal support from experienced clinicians. It also helps you keep on track with prescribed medication through reminders and tracking.

Cardiac Rehabilitation is designed for Westfund members who have eligible hospital cover* and have had an in-patient cardiac event within the past 30 days and have not already participated in the Cardiac Rehabilitation Program. This 6-week evidence-based program** is designed for individuals recovering from a recent heart-related event. It adheres to Australian Heart Foundation and Australian Cardiac Rehabilitation Association guidelines, complementing GP and specialist care.

**This helps you keep on track with prescribed medication requirements and tracking. Members will receive a free blood pressure monitor as part of the program to support monitoring.

*Eligibility criteria

To be eligible, Participants must:

 

  • Be 18 years of age or over;
  • Currently hold a Westfund Hospital Product, have served all waiting periods and are financial;
  • Provide explicit written or verbal informed consent prior to enrolment;
  • Be likely to respond to, and is able to actively participate in, the Services provided under the Program;
  • Is not excluded by one or more of the Program Exclusions; and


 
For the Cardihab Primary Prevention Program, Participants must meet the following clinical inclusion criteria:

  • Considered to be intermediate or high risk by a Heart Health Check or equivalent screening tool; or
  • Have two or more modifiable risk factors, including biomedical (hypertension, hypercholesterolemia, hyperglycemia) and/or lifestyle (current smoker, excessive alcohol, physical inactivity, poor nutrition, overweight/obesity).

 
For the Cardihab Cardiac Rehabilitation Program, Participants must meet the following clinical inclusion criteria:

  • Cardiac rehabilitation referral from treating Cardiologist/Specialist or General Practitioner; or
  • Managing cardiovascular disease with a recent acute presentation/event or cardiac procedure in the past 30-days; or
  • Assessed by hospital staff as safe for discharge to home and/or community setting (if referred from hospital setting).
     

Members must also be on one of the following hospital covers:

  • Gold Tier Hospital Products
  • Silver Plus Assure
  • Silver Plus Nurture
  • Silver
  • Athlete Gold
  • Athlete Silver Plus
  • Overseas Top Plus Hospital
  • Overseas Top Hospital
  • Overseas Hospital

A participant is eligible to take part in any program only once in their lifetime.
